How errors in medicine occur

What seems like a simple order is actually very complex, and involves a $7 million dollar robot

Medicine is comparable to aviation. In fact many of the safety procedures and terminology in medicine come from the field of aviation. Crew resource management, root cause analysis, etc. Here is an interesting article illustrating that reliance on computer systems may be causing more harm than help. Especially if the backup, humans, are no longer able to fly the plane due to under exposure to its controls.
